Some are available as an MS Word template to be downloaded via a browser such as Internet Explorer, Netscape, or Safari; others as both an MS Word template and as a interactive, web-based fill-in which downloads the completed form as an MS Word-compatible Rich Text format (RTF) document ready for printing and signatures.
To be valid, invention and copyright disclosures must be complete and signed by all contributors, by their department head(s), and by witnesses, and delivered to OTT in original paper (not FAX or electronic) format. Any of the other forms listed below must be signed by an authorized individual at the University of Arizona. This is usually the Director of the Office of Technology Transfer, the Director of the Office of Research and Contract Analysis, or another individual designated in writing by the Vice President for Research.
